Please submit proof of eligibility to request or continue receiving this discount as part of our annual re-certification. You can submit the document below, or you can text a picture of the document to 1-877-621-2600. Proof of eligibility options include:

  • Paystub (dated in the last 90 days) 
  • Current ID Badge (with expiration date) for Hospital, USD, Fire Dept, Police Dept, EMT, State/Gov ID Badge. Badges without a date do not qualify for proof of employment.
  • Kansas Farm Bureau membership card with current date or insurance card with current date.

If desired, you may redact or black out any personal information included on these documents. However, please make sure your name, the name of your business/organization, and the date are still visible.
